Guidelines for Selecting HIV Positive Patients for Genital Reconstructive Surgery

Kirk, M.D., Sheila Surgeon TSMC Center, Pittsburgh, Pennsylvania, USA

With the increasing incidence of HIV positive transgendered individuals in the past ten years, it is axiomatic that there be an increase in the numbers of that population who apply for surgery to reconstruct their anatomy, primarily genital but occasionally cosmetic alteration of the face and cranium.

Few surgeons are willing to help these individuals reach their goals for a variety of reasons. But mostly because they don’t understand the process for selection and the potential for great success with relatively little risk or complication. Newer anti-retroviral drug regimens makes it very important to accept these individuals for care because of increased life span expectancy and improved quality of life because of these newer modes of treatment.

This paper discusses the important areas to investigate for selection. It stresses the interchange between HIV management physicians and potential surgeons. HIV positive patients can be treated surgically with excellent outcome and low risk of complications with knowledge of the process.
