The three prime goals of Male to Female Transsexual Surgery are:

  1. Excellent cosmetic result
  2. Effortless, trouble-free function
  3. Orgasmic capability.

Achievement of the first goal requires the reproduction of every structure of the genetic female genitalia especially the trial of:

  1. The clitoris
  2. Visible mucosa from the urethral opening to the superior aspect of the clitoris
  3. The labia minora.

Absence of any one of these structures compromises the authentic appearance of the final result. The importance and the method of formation of the labia minora will be discussed and examples of results with and without the triad will be shown.
