Phalloplasty with Suprapubic Flat – Technical Aspects – Personal Experience

The authors describe their experience with regard to 29 cases of phalloplasty performed since 1997 at the "Servizio de Adeguamento tra Identita Fisica e Identita Psichica (SAIFIP)", of San Camillo Hospital, Rome. The paper illustrates the techniques used and the timing according to the following scheme:

1st step

preparatory

abdominal skin expanders insertion to widen and autonomise the flaps

2nd step

40-60 days later

hysterosalpingo-oophorectomy, following Pryor phalloplasty with suprapubic flap, urethroplasty with inguinal flap (pars mobilis) and supraclitoral flap (pars fixa)

3rd step

3 months later

neo-urethra and original meatus anastomosis

4th step

3 months later

scrotoplasty and prosthesis insertion to obtain neophallus rigidity.

Comparative considerations between this method and the one that employs antebrachial free flap, performed by the authors in 32 cases since 1992, are made.
