This article is a brief historical review and update of the status of phalloplasty. Pertinent literature review has been conducted. Illustrations accompany a short cookbook "How To: From Vagina to Penis" describing the technical feat. The groin flap has been progressively modified in form and function. The clinical results (urologically and erotically) are summarized, including patient satisfaction.
